Patient Information

General Information
  • Confidentiality

    All patients consultations and records are kept strictly confidential and cannot be discussed or reproduced. It is the policy of this practice to maintain security of personal health information at all times and to ensure that this information is only available to authorized members of staff.

  • Home Visits

    For logistical reasons home visits are only offered when it is very difficult for the sick person to come to the surgery. These are at the doctors discretion. Please discuss with your regular GP if you feel a visit is required

  • Telephone Calls

    We do not routinely offer Telephone Consultations and we encourage all contacts with your doctor to be face to face. However please discuss your individual needs with your regular doctor. Should you wish to leave a message for your doctor please inform reception.

    Urgent situations are dealt with immediately, so please advise the reception staff if your call is urgent.

  • Appointments

    All the doctors practicing at our practice run an appointment system for consultations. This assists in keeping the practice running to time. Sometimes there are inevitable delays and you maybe kept waiting. We all apologize for this.

    IF, WHEN MAKING YOUR APPOINTMENT, YOU FEEL THAT YOUR PROBLEM MAY TAKE LONGER THAN THE STANDARD APPOINTMENT TIME, (10-15 mins), IT WOULD HELP IF YOU COULD LET THE RECEPTIONIST KNOW AT THE TIME OF BOOKING. Some examples of this type of extended consultation could be life insurance medicals, driving license checks, wart or mole removal, heart (ECG) checks and emotional problems etc. Of course, we always make provision and time for URGENT consultations, particularly for sick children. Unfortunately, we cannot guarantee that this urgent consultation will be with the doctor of YOUR choice, but we will make EVERY attempt to have a doctor available.

  • Medical Certificates

    Medical certificates are legal documents. They are available only if you are present for a consultation with one of our doctors, we cannot issue medical certificates without you actually seeing the doctor, and we are not permitted to issue certificates retrospectively.

  • Test Results

    Unless organized otherwise with your doctor, please make an appointment to discuss results of tests. You are able to ring for the results of urine samples, Pap tests and pregnancy tests.

  • Patient Information

    It is of the utmost importance that we have your latest up to date medical details, please inform the reception staff when you change Address, Phone Number, Mobile Number, Medicare & Concession Card details. It is very difficult to notify patients if we need to change appointment times when these details are incorrect.

  • Interpreter Service

    We use TIS Telephone Interpreters when necessary.

  • Recalls

    The surgery has a recall system that enables us to send reminders for scheduled preventative care – PAP smears etc. We also use thsi sytem to ensure patients have discussed important results with their doctor.

    Please let us know if you do not wish to be included in our recall register.

     

  • Privacy Information

    This practice complies with the Privacy Act . A copy of the Privacy Policy is in a practice information.

  • Smoking Policy

    It is the policy of the Family Health Group that we are a non smoking workplace.

  • Patient Feedback

    If you are unhappy about any aspect of our services, please feel free to contact us and air your grievances. Your first line of contact should be Kerri, our practice manager, on 8289 1222.

    We also like to receive positive feedback to encourage great service.

  • Code Of Ethics

    The Royal Australian College of General Practitioners defines “GENERAL PRACTICE as: “the provision of primary, continuing, comprehensive, whole patient care to individuals, families and their community”. We feel that our service fits comfortably within these guidelines.

Appointments Info

An appointment is required to be seen at the surgery. Please call our receptionist to make an appointment. We do not accept walk in patients, unless it is a medical emergency. We keep a number of appointments open each day for patients who are sick and need to be seen quickly. To get access to these appointments please ring our receptionist at 8.00am.

  • Double Appointments

    Longer consultation times are also available if you have more than one issue to discuss. Chronic Disease  and Mental Health Care Plans, Drivers Medicals and Work Medicals require a double appointment so please let reception know when booking.

Medical Record
  • Your Personal Health Information

    Family Health preserves the privacy and confidentiality of your personal health information, which includes your medical records, family information, address, employment demographic and account data. Your medical records contain all of your important medical history, such as conditions, treatment, medications, allergies, vaccinations, tests and results. Family Health considers all patient medical records in the strictest confidence between you, the patient, and your doctor. Family Health will never release your medical records without your written consent, unless the records are subponeaed by a court of law.

  • Release of Medical Information

    New patients to us may wish to authorize the release of their medical records from other doctors. This will enable us to provide proper continuity of care for existing medical conditions. Ask your doctor today about how you can request your medical records to be transferred from your old health care provider to Family Health.

  • Transfer of Medical Records to another Health Provider

    Family Health provides third party health care providers with a summary of your medical history to ensure continuity of care, such as when we refer you to a specialist. Family Health will respond to requests to transfer a copy of your medical records to another practice. The request may be submitted to Family Health by the new practice on behalf of you, the patient.

  • Access to Personal Medical Records

    Under the Privacy Act 1988 (Cth) you have the right to access your personal health records from Family Health. If you would like to request a summary or copy of your medical records, please speak with your doctor. Please be aware that administrative fees may apply, and requests are generally processed between 5-10 working days.

  • Your eHealth Record

    Patients may provide their doctor at Family Health with consent to access their eHealth Record. This will enable your doctor to download past health summary, or to upload a new Shared Health Summary or Event Summary to your individual eHealth Record.

    Things your should know about your eHealth Record

    A. You can nominate which healthcare providers you want to access your eHealth Record
    B. You control the types of information accessible by authorized healthcare providers
    C. Only in the event of a medical emergency, where you are unconscious or unable to communicate, may a non-authorized healthcare provider (e.g. doctor within an emergency department) gain temporary access to your eHealth Record
    D. Your health information that is downloaded from your eHealth Record is subsequently stored within Family Health’s secure electronic patient medical record management system

Billing Information
  • Consultation Fees (Elizabeth and Paralowie)

    We offer Medicare On Line Claiming so any Medicare Rebate can be paid directly to your bank account immediately, as long as you have a savings account card.

    • Consultations are bulk billed.
    • Procedures are subject to a fee of $15 for Treatment Room costs.
    • Mirena insertion gap charge is $40 (total cost $157.20, Medicare rebate $117.20)

    PLEASE CALL US IF YOU CANNOT ATTEND YOUR APPOINTMENT.

    We work hard to provide our patients with exceptional care. If you cannot attend your appointment, please call us to cancel it so we can make that time available to another patient needing our help. Fees charged for unattended appointments will be charged as follows:

    • $10.00 fee for a standard appointment not cancelled within two hours of the scheduled standard appointment time.
    • $20.00 fee for a care plan appointment not cancelled within 24 hours prior to the appointment.

    Please understand we assign highly trained nurses and doctors for these appointments and there are many other patients who would appreciate this time. With appropriate notice, we can refill your appointment someone else.

  • Consultation Fees (Golden Grove and Greenwith)

    Golden Grove and Greenwith Family Health are mixed billing practices. Consultation fees will depend on the length and complexity of the consult. Medical consumables associated with the treatment provided will attract a separate fee. All outstanding accounts are to be settled at the time of consultation.

    We offer Medicare On Line Claiming so any Medicare Rebate can be paid directly to your bank account immediately, as long as you have a savings account card.

    Between 8am and 6pm, Monday to Saturday:

    • Standard consultation $47.05, Medicare rebate $37.05, gap payable $10
    • Long consultation $81.70, Medicare rebate $71.70, gap payable $10
    • Children under 16 years, pension and concession card holders are bulk billed

     

    Between 6pm and 8pm:

    • Standard consultation $57.05, Medicare rebate $37.05, gap payable $20
    • Long consultation $91.70, Medicare rebate $71.70, gap payable $20
    • Children under 16 years are bulk billed

     

    Between 8pm and 11pm, and on Sundays:

    • Standard consultation $69.00, Medicare rebate $49.00, gap payable $20
    • Long consultation $103.95, Medicare rebate $83.95, gap payable $20
    • Children under 16 years are bulk billed

     

    Additional fees:

    • Procedures, such as skin lesion removal, are subject to a set additional fee of $20 for treatment room costs.
    • Mirena insertion fee $167.20, Medicare rebate $117.20, gap payable $50.
